Pathophysiology

• Primary mechanism: Benign Prostatic Hyperplasia (BPH) is primarily driven by hormonal changes, particularly the increased conversion of testosterone to dihydrotestosterone (DHT) in the prostate. DHT stimulates prostate cell growth, leading to enlargement of the prostate gland.

• Secondary mechanism: The age-related alteration in the balance of sex hormones, including decreased testosterone and relatively increased estrogen, further promotes prostate cell proliferation and accumulation of stromal and epithelial cells.

• Key complication: The enlarged prostate compresses the urethra, causing urinary obstruction. This can lead to complications such as increased urinary frequency, urgency, nocturia, and, if untreated, can progress to bladder dysfunction or urinary tract infections.
